Do you remember to keep track of your EC?
– Your EC says something about the amount of nutrient salts, what it measures is how well your fertilizer mixture conducts electricity.
If you live in an area with very hard water, this will mean that you already have a relatively high EC before you add your fertilizer, which means you have a smaller gap before you reach the maximum level that your plants can tolerate.
